Long Term Conditions Annual Recall

We have reviewed the way we deliver annual Long Term Condition monitoring. From November 2023 we will be transitioning to a recall date based on your birth month. Please note that if you do not respond to this invite we will not send any further reminders until your next Annual Review is due. In addition we are now offering patients the option to send us their health information for long term conditions online. When your next annual review is due you may be sent a link from NHS A&A Inhealthcare to complete. Please ensure your mobile and email details are up to date which you can do through our website.
